It is common knowledge that the majority of startups fail.  The reasons vary based on who you talk to.  Entrepreneurs often sight running out of money; potential investors say it was the lack of traction or market size, and others say it is the lack of product-market fit.  All of these are valid and related.  Digging deeper, there appear to be three more basic underlying issues that are well within the control of the entrepreneurs that, after the fact, with hindsight, are obvious and could have been avoided.  This article provides the Presentation Notes.
